1. Documentation for ICT Product Conformity Declaration
a) Declaration of conformity document.
b) For entities without a business registration number or individuals lacking a personal identification number, include a copy of one of the following: Business registration certificate; Establishment decision/license, Investment certificate/license (for entities without a business registration number); Identification card/passport (for individuals without a personal identification number).
c) Conformity seal template for the first declaration or when there are changes.
d) For products requiring certification and conformity declaration: Copy of the conformity certificate issued to the manufacturing entity.
đ) For products mandatorily requiring conformity declaration:
đ.1. Self-assessment report, including:
Organization/individual name, address, phone, and fax.
Product name and model.
Manufacturer.
Technical standard code.
Results and date of product testing, certified by an authorized testing unit, serving conformity declaration activities as per Clause 2, Article 5 of this Decree, issued to the organization/individual conducting the self-assessment report.
Conclusion on the product’s compliance with technical standards.
Commitment to product quality compliance with technical standards, applicable disclosure standards, and full legal responsibility for product quality, conformity, and self-assessment results.
đ.2. Technical documentation in Vietnamese or English, including Product name, model, technical details, external photos, and manufacturer information.
Procedure for Conformity Declaration of Domestic ICT Products
a) For incomplete conformity declaration dossiers, within two working days of receiving the dossier, the Telecommunications Department shall notify in writing to supplement the required documents. If not fully supplemented within 15 working days from the date of the written notification, the Telecommunications Department will terminate the processing of the dossier.
b) For complete conformity declaration dossiers, within five working days of receiving the dossier, the Telecommunications Department will verify the validity of the dossier:
b.1. If the dossier is complete and valid, the Telecommunications Department will issue a notice of acceptance of the conformity declaration dossier to the declaring organization/individual.
b.2. If the dossier is complete but invalid, the Telecommunications Department will notify the declaring organization/individual in writing of the reasons for not accepting the dossier.
